Initial day confinement benefit (admission benefit)

This optional benefit provides a significantly higher benefit for the first day of a hospital admission. HSA-compatible.2


Wellness/preventive care benefit

Pays a per-day benefit for routine/preventive screenings like a physical exam or mammogram.


Hospital observation benefit

Pays a benefit for a hospital bed and periodic monitoring for a period of less than 24 hours.


Surgical procedure benefits

Pays a specified benefit for a surgery done in an inpatient or outpatient surgical facility, or at a doctor’s office.


Ground and air ambulance transportation

Pays a benefit for ground ambulance transportation, or air ambulance transportation at double the ground transport amount.


Emergency room benefit

Pays a benefit if the insured is treated at the emergency room and then released.


Outpatient care benefits

Pays a specified benefit for the following, even when the insured isn’t admitted to a hospital:

  • Wellness exams and screening tests
  • Medical imaging (X-rays, MRI or CT)
  • Physical therapy, cardiac rehab, occupational therapy or chiropractic care

1 Insureds must enroll a child as a dependent on their plan within 30 days of birth in order for coverage to continue beyond the first 31 days. In Washington newborn and newly adopted child coverage is automatic for the first 60 days. For coverage to continue, the insured must notify Symetra of the birth or adoption and pay the required premium within the 60 days.
